Lutheran Social Services of the South, Inc. dba Upbring Equal Employment Opportunity/M/F/disability/protected veteran status Job ID 2019-3420 US-TX-Katy Type Regular Full-Time Shift/Hours 1st Shift # of Openings Remaining 1 Overview The Residential Nurse provides direct nursing care to residents, such as administering and ordering medication, conducting medical assessments and diagnostic testing, and medical treatment as prescribed by a licensed medical or clinical professional in accordance with Agency policies, licensing and funding standards, and other federal, state, and local regulatory requirements. Responsibilities Essential Duties * Provide resident with routine health screenings and perform basic health assessment; assess each resident's unique medical needs and ensure they are properly met in a timely manner; report routine medical needs to the medical and clinical staff per Agency policy and licensing standards * Administer medication and treatment to resident as prescribed or as needed in accordance with licensing standards and nursing practices; coordinate medical appointments and follow through on medications and treatment as prescribed; coordinate and oversee the prescribed nutritional needs and diet and fluid intake for resident as prescribed * Keep medication files and maintain inventory of all prescribed medications; order medication and supplies as needed * Assist with the assessment, development, and evaluation of a resident individualized service plan based on resident needs and medical condition; use care plans as a basis for providing therapeutic care to resident * Monitor resident response to treatment plans and identify adverse medication interactions and notify Clinical Director and other clinical and medical staff as deemed appropriate * Create, update and maintain accurate and up-to-date resident case files and records with information pertaining to nursing care, child's medical history, treatment plans, and observations; chart all pertinent information in the resident case files/records in accordance to Agency policy and licensing standards * Work closely with clinical and program team to admit, transfer and discharge resident as directed * Support and enforce infection-control policies and procedures * Use assessment information and resident care during the shift to develop a care plan that accurately communicates the required and needed information for incoming shift to properly and adequately care for the resident * Respond to all crisis situations in a timely manner; ensure all internal and external parties are properly notified in a timely manner; report incidents according to licensing standards * Seek guidance when encountering nursing procedures and responsibilities that have not been mastered * Assist with the safe and physical evacuation of resident during an emergency situation or natural disaster * Provide on-call services when needed * Complete insurance forms and other paperwork as it relates to treatment plans, placement and discharge summaries and administration of medication * Complete monthly, quarterly and annual reports as requested and to remain in compliance with licensing and funding standards * Plan, schedule, and facilitate trainings for staff, such as blood borne pathogens, CPR and First Aid * Provide on-call services when needed * Other duties and special projects as assigned Qualifications Minimum Qualifications * Current LVN/RN license to practice in the state of Texas * 1-year nursing experience * 1 year supervisory or management experience * Ability to maintain a professional approach and build a rapport and trust with residents and their families * Consistently follows established standards, policies, and procedures in providing nursing care * Strong writing skills with the ability to document and maintain accurate resident files/records * Strong customer service, interpersonal, and people skills; ability to wo
